Animal Surrender

Important animal surrender information:
 

To surrender a pet, please review all of the information below before completing the form to help us provide the best service possible. ​

• A $35 surrender fee is a requested donation fee at the time of intake; this helps the MEHS cover the cost of intake.​

​

• Allow at least 30 minutes to meet with a member of our intake staff team. A portion of this time will be used as a preliminary medical exam (including blood/fecal testing). If any medical concerns arise from this exam, that MEHS is not prepared to treat, the appointment may be canceled or postponed.

• Bring any previous veterinary and vaccination records, if available.

• Items such as favorite toys, food, and bedding are welcome but may not remain with your pet throughout their entire stay.
